AN ASSESSMENT OF SELF CARE PRACTICES AND ASSOCIATED FACTORS IN THE MANAGEMENT OF HYPERTENSION AMONG CLIENTS LIVING WITH HYPERTENSION AT THE NKWEN DISTRICT HOSPITAL.
Abstract
Hypertension is a condition that affects most of the people throughout the world and is a leading cause of morbidity and death. Hypertension related knowledge and practice such as life style modification, physical activity; nutrition etc. plays an important role in controlling and preventing its long term complications of hypertension. This study aims at assessing the self-care practices and associated factors in the management of hypertension among clients living with hypertension at the Nkwen District Hospital.
Specific objectives
To assess the client’s self care practices in the management of hypertension at the Nkwen District Hospital.
To find out the factors associated to the self management of hypertension at the Nkwen District Hospital.
Method
A hospital based descriptive cross sectional study was used and structure questionnaires were used to collect data on self care practices and associated factors in the management of hypertension among clients living with hypertension at Nkwen District Hospital. A convenient sampling technique was used. A total of 43 participants were included in the study. Data collected was analyzed using excel 2016 and word document.
Results
The study found out that take medications as prescribed and cessation from smoking was the major self care practice in the management of hypertension. While the presence of social and financial sustainability promote the self care management of hypertension and lack of social and family support, limited knowledge of the disease condition limit the self management of hypertension.
Conclusion
The presence of social support and financial sustainability promote the self management of hypertension while lack of social and family support, limited knowledge of the disease condition hinders the self management of hypertension.
Recommendation
The ministry of public health should provide adequate funds in order to support the institution to manage clients with hypertension.

1.1. Background
Globally, one third of adults have hypertension and by 2025, the prevalence of hypertension among the population is predicted to be 29 % (1.56billion) (Berhen et al, 2018). Three fourth of them resides in low and middle income countries where resources are limited, patients have little knowledge and poor control over the condition (Kim-shin et al; 2016).
Globally, the burden of disease attributable to hypertension has significantly increased from 4.5% in 2000 to 7% in 2010. This makes hypertension as the single most common cause of morbidity, and mortality in the world and this high lights the urgent need for action to address the problem (Hassenabdi et al, 2020).
Hypertension is a chronic non communicable disease with high blood global health challenges, due to its high prevalence, and the resulting cardiovascular stroke and chronic renal complications. It is the third leading preventable risk factor for premature death and disability worldwide.
Self-care practice describes a conscious act one takes in order to promote their own physical, mental and emotional health. There are many forms of self-care practice to undertake. It could be ensuring you get enough sleep every night or stepping outside for a few minutes for some fresh air. Self-care practice is the activity that hypertensive patient undertakes intending to improve their health (hassen Abdi et al 2020) poor self-are practice leads to uncontrolled hypertension. Therefore, strategies designed to prevent and control hypertension should consider the level of patient’s self-care practice and risk factors.
In sub Saharan Africa the number of hypertension cases is projected to be between 125.5 and 162.8 million by 2025 (Nazrul Mallick et al, 2017). Though the exact cause of hypertension is unknown, evidence indicate modifiable factors such as harmful use of alcohol, physical activity, high salt intake, tobaccos and higher body mass index increase the risk of hypertension. The avoiding of these factors is the critical component of self-care practice, which is considered an important activity undertaken by individuals in order to improve health or prevent disease. Self-care practice is a non-pharmacologic strategy that not only reduces the blood pressure but also the risk of other cardiovascular risk factors. (Bezalumengistie et al., 2020).
In Cameroon hypertension remains a leading cause of the global burden of disease (jean Claude et al 2019) over 74 % of African countries have no guidelines for the management of hypertension.
Hypertension prevention and control strategy requires joint efforts not only from the patients and families but also from the governments and policy makers. Hypertensive patients should start implementing self-care practices before initiating prescribed medications and must continue thereafter (Dasgupta et al, 2018). Hence, compliance with self-care practices helps and reduces complications and subsequent morbidities mortality and death.
Previous studies (Gerald albert et al, 2019) showed that sex, age, occupational status and time since diagnosis, comorbidities, knowledge about self-care agency, social support, smoking and tobacco chewing are associated with components of self-care practice of hypertension yet there is no clear comprehension of the magnitude of self-care practices and the factors that influence the self-management practice of hypertensive patients.
Also, lack of income, lack of knowledge, social support, location of resident and time since diagnosis can limit the self- care practices in the management of hypertension. Thus the study is aimed at assessing self- care practices and associated factors in the management of hypertension among clients living with hypertension at the Nkwen District Hospital.
