EFFECT OF THE MODE OF ANESTHESIA IN MATERNAL AND FOETAL SHORT TERM OUTCOMES AFTER CESAREAN DELIVERY
Abstract
General anaesthesia (GA), spinal anaesthesia (SA), epidural anaesthesia (EA) and local anaesthesia (LA) are anaesthetic techniques for cesarean delivery. These anaesthetic techniques have their effects on both the foetus and mothers. The choice of anaesthetic techniques for cesarean delivery depends on several factors including indications, experiences of the anaesthesiologists and practitioner, availability of drugs, and equipment, among others. However, whichever technique is used, it is because of the safety profile and benefit to both mother and foetus. There is a paucity of data on the effects of anaesthesia in maternal and foetal outcomes after cesarean delivery in South West region.
This study aimed to assess the effects of the mode of anaesthesia in maternal and foetal outcomes after cesarean delivery at Buea and Limbe Regional Hospitals.
This was a hospital based cross-sectional study at the maternity and theater departments of Buea and Limbe Regional Hospitals. Data collection was done using a well-structured questionnaire. Data collected was entered using the Statistical Package for Social Sciences, SPSS version 23. Continues variable was computed as means and standard deviation whereas categorical variables as proportions and percentages.
They was similarities between the mean age and mean gestational ages of women who benefited general and spinal anaesthesia 30.1 (± 5.14) years; (38.20± 2.11) weeks. The intraoperative hypotension is higher in spinal anaesthesia compared to general anaesthesia (39 (69.6%) Vs 6 (7.2%) p-value <0.001), postoperative hypertension was higher in general
compared to spinal anaesthesia (9 (16.1%) Vs 5 (6.0), p-value= 0.054). The mean of interval of extraction of the new born was low in general compared with spinal anaesthesia (3.25+2.46 Vs 6.80+4.17) P-value<0.001. In other hand the mean APGAR score > 7 in spinal anaesthesia was higher compared to general anaesthesia at one and fifth minutes (8.23+0.85 Vs 7.58+1.31), P- value<0.001. Neonatal asphyxia was find in GA compared to SA.
In conclusion, spinal anaesthesia emerges as a better option evidenced by its better foetal and maternal. However, both spinal anaesthesia and general anaesthesia are still used for cesarean delivery.
