DETERMINANTS OF HIV/AIDS IN PREGNANT AND BREASTFEEDING WOMEN ATTENDING ANC IN YAOUNDÉ
Abstract
The Human Immunodeficiency Virus (HIV) is an enveloped retrovirus that contains 2 copies of a single-stranded RNA genome which mainly invades the immune system (particularly on CD4+ T lymphocytes, monocyte macrophages, and dendritic cells). Due to the ravaging nature of this disease, numerous organizations and institutions have been set up with a mandate to fight against this infection.
In Cameroon in particular, intensive measures have been put in place by the government and other institutions to curb the spread. Few studies have so far been done to assess the prevalence, co-infection rate and associated risk factors of this disease among pregnant women in the Yaoundé Health District.
A hospital based cross-sectional and retrospective study was used in this study. For the retrospective a data extraction was used to collect data. For the Cross-sectional study, a multi-stage sampling technique was used to sample 484 participants. Data was analyzed using Statistical Package for Social Sciences version 26. Descriptive statistics were used to summarize the data. The Chi-square test was used to determine association between HIV/AIDS and categorical variables like age group. Logistic regression was used to determine the final factors associated to HIV/AIDS in pregnant and breastfeeding women.
A total of 4441 files were included in the retrospective study. A total number of 2421 (54.5%) of the files were from the CNPS hospital and 2020 (45.5%) were from Biyam-Assi. The overall prevalence of HIV from the review of records from 202-2021 was 163 (3.7%). The prevalence was 75 (3.1%) and 88 (4.3%) in the CNPS and Biyem-Assi District hospitals respectively and was found to be significantly associated with health facilities, age group, and number of abortions.
For the cross-sectional study, the prevalence was found to be 38 (7.8%) and was found to be significantly associated with health facilities, age group and level of education. About 109 (22.5%) of respondents had correct knowledge on mother-to-child transmission of HIV/AIDS and 84 (17.4%) of participants had correct knowledge on mother-to-child prevention of HIV/AIDS. Health facilities (aOR = 23.0, P < 0.001) and age group (aOR = 13.115, P = 0.003) were found to be significant determinants of HIV/AIDS among pregnant and breastfeeding women attending ANC at the Yaoundé Health District.
The retrospective prevalence of HIV/AIDS was 163 (3.7%), that of the cross-sectional was 38 (7.8%). Health facilities and age group were found to be significant risk factors of HIV/AIDS among pregnant and breastfeeding women attending ANC at Biyam-Assi and Essos health facilities. knowledge on transmission and prevention was 109 (22.5%) and 84 (17.4%) respectively. prevalence of HIV/AIDS was lower than reported in a past study. The knowledge of pregnant and breastfeeding women on HIV/AIDS was moderately high. Age, level of education and health facility were significant factors associated with HIV/AIDS.
