PREVALENCE AND ASSOCIATED FACTORS OF ACQUIRED CYSTIC KIDNEY DISEASE IN PATIENTS ON MAINTENANCE HEMODIALYSIS
Abstract
Background: Acquired Cystic Kidney Disease (ACKD) is a well-known complication in hemodialysis patients, and it is associated with a high chance of malignant transformation. In Sub-Saharan Africa, there is a scarcity of data on ACKD.
Objectives: To determine the prevalence and factors associated with Acquired cystic kidney disease in patients on maintenance hemodialysis in Yaounde.
Materials and Method: During a three-month period, all consenting patients on maintenance hemodialysis for at least three months at two referral hospitals in Yaounde were tested for ACKD. Patients with hereditary cystic kidney disease were barred from participating. Two radiologists used NEMIO MX R Toshiba, Japan Care, 2011 and ARIETTA V70 R Hittachi Aloka, Healthcare Americas 2016 ultrasound equipment to do renal ultrasounds. Cysts were seen in the subcostal, ventral, and dorsal positions.
ACKD was described as the presence of three or more bilateral renal cysts in a small or normal-sized kidney. The International Business Machines Statistical Package for Sciences (IBM-SPSS) version 22.0 was used to analyze the data. The associated variables were evaluated using logistic regression with a p-value of 0.05 as the threshold of significance.
Results: A total of 158 participants were enrolled of which 61.4% (97) were male. Their mean (SD) age was 45.8(14.9) years with 58.9% (93) aged bellow 50 years. The median [25th-75th percentile] dialysis vintage was 33.5[10.7-63.2] months.
The mean (SD) pole to pole length of the kidneys was 85.1(17.5) mm on the left and 81.2(17.1) mm on the right. Renal cysts were seen in 63.3%(n=100) of the participants of which 43%(n=69) were bilateral.
The prevalence of ACKD was 31.6% (n=50), with one patient having a superimposed renal tumor. In individuals with acquired cystic kidney disease, septated cysts (4), calcification of the cyst wall (2), irregular thick calcific wall (1), septated cysts with calcification (1), and hemorrhagic cyst (1) cysts were also found. Dialysis vintage >36 months (OR 7.1 (95% CI 3.3 – 15.5), p0.001) and male sex (OR 2.6 (95% CI 1.2-5.6), p=0.012) were linked with ACKD separately.
Conclusion: The frequency of ACKD is significant in a Cameroonian population on maintenance. This finding necessitates the introduction of screening techniques for the illness and its complications.
